Formula
Total Out-of-Pocket = (Monthly Premium × 12) + Min(Deductible + (Covered Claims - Deductible) × Coinsurance %, OOP Max)
Worked Example
Monthly premium $300 ($3,600/yr), Deductible $2,000, Coinsurance 20%, Out-of-pocket max $5,000. For $10,000 in covered medical claims: Out-of-pocket = $2,000 + ($8,000 × 20% = $1,600) = $3,600 medical + $3,600 premium = $7,200 total annual cost.

- All medical treatments and prescriptions are in-network covered services.
- Deductible resets annually at policy renewal.
Limitations
- Excludes out-of-network balance billing, non-covered elective procedures, and tier-based prescription copay variances.
